Keshet, Confirmation, and Davis Reform Temple Youth (DRTY) programs, for Teens from Partner and non-Partner households.
Please note that this form alone does not suffice as enrollment in Teen Programming. This form AND the 2020-21 Emergency Medical Information Form, which you will see a link to when you press the gold “Submit” button at the bottom of this form must both be received for your Teen’s enrollment to be complete.
We appreciate your perseverance in filling out each of the forms. A link to the Emergency Medical Form will appear as part of the confirmation message when you submit this form.
