• This form is intended to be used by you as a substitute for your personal appearance at the upcoming CBH 2022-23 Annual Meeting, scheduled to be held on June 12, 2022, at 1:00 P.M. At that meeting, CBH partners will be asked to vote on whether to elect nominees for seven (7) open seats on the CBH Board of Directors.

This election cannot occur unless at least 25% of the CBH partnership units are represented at the meeting, either by attending in person or by submitting this proxy form. Therefore, if you are unable to attend the meeting, it is essential that you complete this form by no later than Thursday, June 9, 2022.

• By placing my name on and returning this form to CBH, I hereby authorize CBH Co-President Mitch Singer to act as my proxy in attending the CBH 2022-23 Annual Meeting on June 12, 2022, and in casting votes on my behalf on the election of nominated candidates to serve three-year terms on the CBH Board of Directors beginning on that date.
